Medical Records
We want to make it as simple as possible for you to get a copy of your medical records, and we are required by law to obtain certain written information prior to processing the request. Please follow the steps below:
For Office Visit Medical Records
Request to Access and/or Copy Protected Health Information Form
To have copies of your medical or billing records from an OFFICE VISIT mailed to you AND you are a patient or the parent/legal guardian of a minor patient, complete the Request to Access and/or Copy Protected Health Information Form and mail or fax it to the division where services were provided.
Authorization to Use and/or Disclose Protected Health Information Form
To have copies of your medical or billing records from an OFFICE VISIT sent to a non-FNA health care provider, insurance company, school, etc., complete the Authorization to Use and/or Disclose Protected Health Information Form and mail or fax it to the division where services were provided.

The Pediatric Lung and Allergy Center is closed as of 10/22/2021. Our Pediatric Pulmonologists have transitioned to the Pediatric Specialists of Virginia. If you need to schedule an appointment, please call 703-876-2788.
To have copies of your medical or billing records from an office visit mailed or faxed to you and you were a patient or the parent/legal guardian of a minor patient, please complete the “request to access and/or copy protected health information form” which can be found here and mail or fax the completed form to us.
