Medical Records

We want to make it as simple as possible for you to get a copy of your medical records, and we are required by law to obtain certain written information prior to processing the request. Please follow the steps below:

For Office Visit Medical Records

To have copies of your medical or billing records from an OFFICE VISIT mailed to you AND you are a patient or the parent/legal guardian of a minor patient: Complete the Request to Access and/or Copy Protected Health Information Form (English | Spanish ) and mail or fax it to the division where services were provided.

To have copies of your medical or billing records from an OFFICE VISIT sent to a non-FNA health care provider, insurance company, school, etc.: Complete the Authorization to Use and/or Disclose Protected Health Information Form (English | Spanish ) and mail or fax it to the division where services were provided.

If you or your child received services at a hospital or facility (inpatient or outpatient), please contact the institution where services were provided to request copies of their medical or billing records.
