I have Blue Cross Blue Shield why do I need to go the Leesburg office for in network benefits?
Blue Cross Blue Shield Association requires healthcare providers to submit to their local plan, determined by service location. The service area set by the Association is determined by services provided either East of Route 123 or services provided West of Route 123. Currently, The Pediatric Lung Center providers do not contract with Carefirst (the local plan for service locations East of Route 123) and services provided at the Prosperity location would be processed at the members out of network level of benefits. The Pediatric Lung Center providers are contracted with Anthem of Virginia (the local plan for service locations West of Route 123) and services provided at the Leesburg location would be processed at the members in network level of benefits.

Do you need to know all of the insurance policies my child will have for coverage?
Yes, in order to ensure accuracy of insurance reimbursement any insurance that your child is enrolled on the date of service requires notification. When you have multiple plans, there are plan benefit rules which determine primary and secondary carriers.

What is coordination of benefits?
Coordination of benefits (COB) is required when you have multiple insurance plans. Coordination of benefits is the determination of which insurance pays: primary, secondary, or tertiary.

How do I request a copy of my medical or billing records?
We want to make it as simple as possible for you to get a copy of your medical and/or billing records, but we are required by law to obtain certain written information. Please follow the steps below:

  • To have copies of your medical or billing records mailed to you personally
    If you are a patient or the parent/legal guardian of a minor patient, complete the Request to Access and/or Copy Protected Health Information Form and mail or fax it to The Pediatric Lung Center. If you or your child were an inpatient, please contact the hospital where services were provided directly to request copies of your medical or billing records.
  • To have copies of your medical or billing records sent to a non-FNA health care provider, insurance company, school, etc.
    Complete the Authorization to Use and/or Disclose Protected Health Information Form and mail or fax it to The Pediatric Lung Center. If you or your child were an inpatient, please contact the hospital where services were provided directly to request copies of your medical or billing records.
